Required Forms
1. Participant Agreement (includes emergency contact information)
Each student must complete the participant agreement (.doc file) on the first day of class. Keep this document for your records for the duration of the trip. Place in the manilla envelope and turn in to the MSSE office at the end of trip. It includes critical information such as how to get in touch with students' emergency contacts.
2. MSU-Bozeman Driver's Agreement
Drivers will need to sign the Driver's Agreement (.doc file) if they are transporting other trip participants in a rented vehicle. This form will be collected by the MSSE office before you leave on your trip.
What to Do in Unusual Situations and Emergencies
1. Steps to Take in Unusual Situations
Incidents resulting in injury, any criminal activity with travelers as victims or perpetrators, property loss, the dismissal of a disruptive participant, and/or other trip rule violations should be reported by the instructor as soon as possible.

2. Steps to Take in an Emergency
Instructors should call the University Police 24-hour dispatch in the event of an emergency during a trip. 406-994-2121.
3. In the event of an accident, get as much information as possible to complete an Incident Report.
